10 Best Cities for Healthcare Access in the US (2026)
Ranked: the 10 best U.S. cities for healthcare access based on provider density, cost, transparency scores, and patient outcomes data.
10 Best Cities for Healthcare Access in the US (2026)#
Healthcare access is not evenly distributed across the United States. Where you live dramatically affects how many providers are available, how much you pay, how long you wait for an appointment, and how transparent pricing is.
We ranked the top 10 metro areas for healthcare access using data from our directory of 3.5 million NPI-verified healthcare providers and our transparency scoring system. The ranking weights four factors equally: provider density, cost relative to income, transparency score, and patient outcomes.
The Rankings#
| Rank | City | Overall Score | Provider Density | Cost Score | Transparency | Outcomes | |---|---|---|---|---|---|---| | 1 | Minneapolis, MN | 92 | 95 | 88 | 90 | 94 | | 2 | Boston, MA | 90 | 97 | 75 | 92 | 96 | | 3 | Salt Lake City, UT | 88 | 85 | 92 | 88 | 87 | | 4 | Denver, CO | 87 | 88 | 84 | 89 | 86 | | 5 | Portland, OR | 86 | 86 | 82 | 90 | 85 | | 6 | Raleigh, NC | 85 | 84 | 90 | 84 | 82 | | 7 | Pittsburgh, PA | 84 | 90 | 88 | 78 | 80 | | 8 | Seattle, WA | 83 | 87 | 74 | 88 | 84 | | 9 | Austin, TX | 82 | 80 | 85 | 83 | 79 | | 10 | Nashville, TN | 81 | 83 | 86 | 76 | 78 |
1. Minneapolis, MN — Overall Score: 92#
Minneapolis earns the top spot by excelling across every dimension. The Twin Cities metro has one of the highest ratios of primary care physicians per capita in the country (1 per 850 residents vs. the national average of 1 per 1,300). Major health systems including Mayo Clinic, Allina Health, and HealthPartners create intense competition that keeps quality high and drives innovation.
Key stats:
- Primary care providers per 100K: 118 (national avg: 77)
- Average primary care visit: $165 (national avg: $210)
- Insurance coverage rate: 95.2% (national avg: 91.4%)
- Average wait for new patient appointment: 12 days
The Minnesota health system also benefits from a strong public health infrastructure and one of the highest insurance coverage rates in the country, thanks to MinnesotaCare and a well-functioning ACA exchange.
2. Boston, MA — Overall Score: 90#
Boston is the undisputed capital of academic medicine. Massachusetts General Hospital, Brigham and Women's, Beth Israel Deaconess, and Dana-Farber Cancer Institute represent some of the best specialized care available anywhere. The concentration of medical schools (Harvard, Tufts, Boston University) creates a deep pipeline of providers.
Key stats:
- Primary care providers per 100K: 132 (highest in the U.S.)
- Average primary care visit: $250 (above national avg)
- Insurance coverage rate: 97.1% (highest in the U.S.)
- Average wait for new patient appointment: 18 days
Boston's weakness is cost. Healthcare prices in the Boston metro are among the highest in the country, driven by the premium pricing of world-class academic medical centers. The state's near-universal insurance coverage helps offset this for most residents.
3. Salt Lake City, UT — Overall Score: 88#
Salt Lake City offers the rare combination of high-quality care at below-average prices. Intermountain Health, one of the most studied health systems in the country for quality and efficiency, anchors the market. The population is younger and healthier on average than most metros, which keeps demand manageable and wait times short.
Key stats:
- Primary care providers per 100K: 88
- Average primary care visit: $155
- Insurance coverage rate: 90.8%
- Average wait for new patient appointment: 8 days
4. Denver, CO — Overall Score: 87#
Denver benefits from a strong economy, a health-conscious population, and a competitive provider market. UCHealth, SCL Health, and Centura Health compete for patients, driving both quality and transparency improvements.
Key stats:
- Primary care providers per 100K: 92
- Average primary care visit: $185
- Insurance coverage rate: 93.1%
- Average wait for new patient appointment: 11 days
5. Portland, OR — Overall Score: 86#
Portland's healthcare market is anchored by OHSU (Oregon Health & Science University), Providence, and Kaiser Permanente. Oregon's progressive health policy environment — including the Oregon Health Plan and early adoption of price transparency measures — gives Portland high marks on accessibility and transparency.
Key stats:
- Primary care providers per 100K: 90
- Average primary care visit: $180
- Insurance coverage rate: 93.8%
- Average wait for new patient appointment: 13 days

Austin, TX (Score: 82) — Rapidly growing healthcare infrastructure matching the city's population boom. Dell Medical School (opened 2016) is improving academic medicine access. Texas pricing keeps costs reasonable.
Nashville, TN (Score: 81) — The "Healthcare Capital" is home to HCA, Vanderbilt, and dozens of health system headquarters. Strong outcomes and affordability, with room for improvement on price transparency.
What the Data Shows About Access Gaps#
Not every city fares well. The metros with the worst healthcare access scores in our data share common characteristics:
- Provider shortages: Rural-adjacent metros and fast-growing cities where population has outpaced provider recruitment
- High uninsured rates: States that have not expanded Medicaid show consistently lower access scores
- Hospital consolidation: Metros where mergers have reduced competition tend to have higher prices and lower transparency
- Mental health deserts: Even in top-ranked cities, mental health provider shortages are acute. The national average wait for a new psychiatry appointment is 45 days.

FAQ#
What does "provider density" measure?#
Provider density is the number of healthcare providers per 100,000 residents in a metro area. We measure this separately for primary care, specialists, and mental health providers. Higher density generally means shorter wait times, more appointment availability, and greater choice.
Why is cost measured relative to income?#
A $200 doctor visit in San Francisco (median household income: $120K) is a very different burden than a $200 visit in Memphis (median household income: $48K). We index healthcare costs against local median income to provide a more meaningful comparison of affordability.
Does insurance coverage rate affect the ranking?#
Yes. Higher insurance coverage rates correlate with better access because insured patients are more likely to seek preventive care, can access a broader network of providers, and face lower financial barriers. States with Medicaid expansion consistently show higher coverage rates and better access scores.
How often is this data updated?#
Our provider directory is continuously updated as NPI registrations change and new transparency data becomes available. This ranking reflects data as of early 2026. We plan to update the city rankings annually.
